Job summary

Qualifications

• Active RN license in good standing (NJ).
• BSN or related health field from an accredited institution.
• Minimum 2 years RN experience in community/home health.
• Ability to document and communicate clearly.

Responsibilities

• Evaluate and document clients' needs and provide direct nursing care.
• Administer medications and perform clinical treatments per Plan of Care.
• Coordinate scheduling for multidisciplinary appointments and therapies.
• Report changes in client condition to the Director of Nursing and physician.
• Adhere to HIPAA, infection control, and safety guidelines.
• Maintain complete client records and timecards; participate in quality improvement.
• Reports to Director of Nursing.
