Qualifications

• Current LVN/LPN license in Kentucky with good standing.
• Minimum 2 years of related experience or an equivalent combination.
• Ability to administer prescribed medications and treatments.
• Familiarity with nursing techniques and care plans.
• Compliance with Federal, State, Local regulations and company policies.

Responsibilities

• Provide total nursing care for residents in the assigned unit.
• Explain procedures and care plans to residents.
• Administer medications and treatments per approved techniques.
• Transcribe physician orders and chart progress notes.
• Receive and relay shift reports; document accurately.
• Respond to emergencies in line with nursing policies.
• Assist in care plan development and participate in care conferences.
• Ensure CNA documentation aligns with the care plan.
• Coordinate rehabilitation with therapists to meet 24-hour needs.
• Address nutrition and safety needs; monitor residents’ condition.
