Job summary

Augusta Health and Rehabilitation is seeking a Licensed Nurse (RN/LPN) for the AM shift on a part-time basis in our skilled nursing and rehabilitation community. You will provide professional nursing care, administer medications, complete assessments, and collaborate with the nursing team to support resident-centered care.

The role requires a current Wisconsin RN or LPN license, strong clinical judgment, and a compassionate, team-oriented approach.

Qualifications

Responsibilities

• Provide professional nursing care to residents in our skilled nursing and rehabilitation community.
• Administer medications and treatments according to physician orders and facility standards.
• Complete assessments, monitor resident conditions, and communicate changes to the appropriate team members.
• Collaborate with CNAs, providers, families, and interdisciplinary team members.
• Support resident care plans and promote resident safety, comfort, and dignity.
• Complete accurate and timely clinical documentation.
• Provide guidance and support to nursing assistants to promote quality resident care.
