Augusta Health and Rehabilitation in Augusta, WI is seeking a dedicated nurse for a full-time position. You will provide professional nursing care to residents in our skilled nursing and rehabilitation community, and administer medications per orders.

The role includes completing assessments, monitoring conditions, and communicating changes to the interdisciplinary team. You will work with CNAs, families, and providers to support individualized resident care.

Qualifications

• Current Wisconsin RN or LPN license (required).
• Strong clinical judgment and critical thinking skills.

Responsibilities

• Provide professional nursing care to residents in our skilled nursing and rehabilitation community.
• Administer medications and treatments according to physician orders and facility standards.
• Complete assessments, monitor resident conditions, and communicate changes to providers and the interdisciplinary team.
• Collaborate with CNAs, families, providers, and team members to support individualized resident care and promote safety, comfort, and dignity.
• Complete accurate and timely clinical documentation.
• Provide direction, support, and leadership to nursing assistants to promote quality resident outcomes.
