Associated Home Care is seeking a part-time Registered Nurse to administer the Care Plan and manage the health and wellness of clients in home settings. The RN will ensure compliance with nursing standards and collaborate with the Director of Nursing and care team to provide high-quality, compassionate care.

The role involves on-site orientation, supervision of home health aides, and ongoing patient condition documentation, with a focus on safety and HIPAA compliance.

Formación

• Graduate of an approved school of professional nursing.
• Current licensed Registered Nurse in practicing state.
• One or more years of experience in clinical home health services, or equivalent preferred.
• Must be a licensed driver in the appropriate state with current insurance.

Responsabilidades

• Follows the Care Plan in collaboration with the Director of Nursing and care team.
• Performs on-site orientation, supervision visits, skill checks, and annual performance reviews for home care aides.
• Provides ongoing instructions to home health aides during field visits, reinforcing safety protocols.
• Reviews Care Plans from ASAP and orients aides to follow the plan as authorized.
• Documents services rendered and changes in client conditions or home situation.
• Communicates with ASAP Case Managers or client’s legal representative as needed.
• Performs other duties as assigned.
