Qualifications

• Active Minnesota RN license required or in process.
• Hospice experience or strong background in end-of-life care preferred.
• Strong clinical assessment, communication, and critical thinking skills.
• Ability to work independently with a flexible field schedule.
• Reliable transportation and valid driver’s license.

Responsibilities

• Travel to patients' homes, assisted living communities, memory care, and skilled nursing facilities for routine hospice visits.
• Perform comprehensive nursing assessments and provide pain and symptom management.
• Deliver wound care, medication management, and comfort-focused nursing interventions.
• Educate and support patients, caregivers, and families throughout the hospice journey.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and the interdisciplinary team.
• Complete accurate EMR documentation while maintaining Medicare hospice compliance.

• Hospice experience preferred, or a strong background in home health, oncology, ICU, palliative care, med-surg, or another clinical setting with end-of-life care experience.
• Strong clinical assessment, communication, and critical thinking skills.
• Ability to work independently while managing a flexible field schedule.
• Reliable transportation and a valid driver's license.
