Job summary

Superior Hospice, part of the Home Health Care, Inc. family, is seeking a compassionate Hospice Visit Nurse (RN) for a part-time role serving patients at home across Minnesota.

You will provide routine hospice visits, conduct assessments, manage symptoms, educate families, and coordinate with physicians and social workers. The position offers a path to full-time as patient volume grows and a supportive, patient‑centered team.

Qualifications

• Minnesota RN license required; hospice experience preferred.
• Field-based nursing in patients' homes or facilities.
• Ability to travel within Minnesota to provide visits.

Responsibilities

• Provide routine and PRN hospice visits in patients' homes and facilities.
• Complete nursing assessments, symptom management, and medication education.
• Support patients and families through education and compassionate communication.
• Collaborate with physicians, social workers, chaplains, and the interdisciplinary team.
• Complete accurate, timely EMR documentation.

What You'll Do

• Complete nursing assessments, symptom management, medication education, and comfort-focused interventions.
• Support patients and families through education, advocacy, and compassionate communication.
• Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate, timely EMR documentation.

What You'll Bring

• Active Minnesota Registered Nurse (RN) license.
• Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, med-surg, or another clinical setting with end-of-life care exposure.
• Excellent assessment, communication, and time‑management skills.
• Ability to work independently in a field-based role.
• Reliable transportation and a valid driver's license.
