Job summary

Community Options, Inc. is seeking a part-time Registered Nurse (RN) to provide skilled nursing care for individuals with disabilities in Mercer County, New Jersey. The role follows the nursing process of assessment, planning, intervention, implementation, and evaluation.

Work schedule is Monday–Friday from 2 p.m. to 6 p.m. with an hourly pay rate of $38.00. The RN will collaborate with an interdisciplinary team to develop and update ISP/PCP plans, monitor health, and document care.

Qualifications

Valid R.N. license.
Valid driver’s license with a satisfactory driving record.
Ability to calculate figures related to medication dosages.
Ability to effectively communicate with program participants, third-party agencies, staff, and members of the community.

Responsibilities

Ensure individuals attend medical appointments per their ISP.
Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations.
Develop and update ISP/PCP with an interdisciplinary team to include concerns, observations, and behavioral developments.
Train employees on the medical needs of assigned individuals.
Monitor health and medical conditions of individuals and refer for medical services as needed.
Monitor the use of medications through medication administration logs.
Ensure program documentation and billable records are completed accurately and timely.
Participate with individuals in community, social, and recreational activities.
Participate in circles of support for personal futures planning.
