Job summary

The BAYADA Home Health Care Essex Assistive Care State Programs office in Bloomfield, NJ seeks an organized RN to lead clinical oversight for clients enrolled in Medicaid waiver programs. This part-time position supervises Home Health Aides and Personal Care Assistants, ensures care hours align with approved parameters, and partners with state case managers to maintain compliance and quality.

Qualifications

• Current Registered Nurse (RN) license in good standing in New Jersey.
• Minimum of 2 years of RN experience.
• Strong organization, documentation, communication, and problem-solving skills.
• Ability to manage state program requirements, client needs, and competing priorities.
• Ability to provide clinical leadership and support to caregivers in a community-based setting.
• Comfort using computer systems for documentation, communication, and care coordination.
• Commitment to serving clients and families with compassion, accountability, and respect.

Responsibilities

• Provide clinical oversight for clients enrolled in state-funded Medicaid waiver and personal care programs
• Conduct client assessments and develop individualized care plans aligned with client needs and state program guidelines
• Supervise, support, and provide clinical guidance to Home Health Aides and Personal Care Assistants
• Ensure care delivery aligns with approved services, authorized care hours, state program requirements, and BAYADA standards
• Coordinate with state case managers, Medicaid managed care organizations, families, caregivers, and other external stakeholders
• Support client reauthorization processes and help ensure care hours remain aligned with approved program parameters
• Review documentation for accuracy, completeness, timeliness, and compliance with applicable state and program requirements
• Facilitate caregiver training, orientation, and competency documentation as required
• Monitor quality of care and respond to incidents, complaints, changes in client condition, and service concerns
• Conduct supervisory visits and follow up on client and caregiver needs
• Serve as a clinical resource, client advocate, and partner to the Director and Client Services Manager
• Participate in quality assurance, performance improvement, and audit-readiness activities
• Build positive relationships with clients, families, caregivers, state partners, and community resources
