Qualifications

• RN license in New Jersey in good standing.
• Minimum of 2 years of RN experience.
• Strong organization, documentation, communication, and problem-solving skills.
• Ability to manage state program requirements, client needs, and competing priorities.
• Ability to provide clinical leadership and support to caregivers in a community-based setting.
• Comfort using computer systems for documentation, communication, and care coordination.
• Commitment to serving clients and families with compassion, accountability, and respect.

Responsibilities

• Provide clinical oversight for clients enrolled in state-funded Medicaid waiver and personal care programs.
• Conduct client assessments and develop individualized care plans aligned with client needs and state program guidelines.
• Supervise, support, and provide clinical guidance to Home Health Aides and Personal Care Assistants.
• Ensure care delivery aligns with approved services, authorized care hours, state program requirements, and BAYADA standards.
• Coordinate with state case managers, Medicaid managed care organizations, families, caregivers, and other external stakeholders.
• Support client reauthorization processes and help ensure care hours remain aligned with approved program parameters.
• Review documentation for accuracy, completeness, timeliness, and compliance with applicable state and program requirements.
• Facilitate caregiver training, orientation, and competency documentation as required.
• Monitor quality of care and respond to incidents, complaints, changes in client condition, and service concerns.
• Conduct supervisory visits and follow up on client and caregiver needs.
• Serve as a clinical resource, client advocate, and partner to the Director and Client Services Manager.
• Participate in quality assurance, performance improvement, and audit-readiness activities.
• Build positive relationships with clients, families, caregivers, state partners, and community resources.
