Qualifications

• Current RN License and CPR certification; 2-5 years in public health, community health, home care or hospital care.
• Ability to develop effective interpersonal relationships with health providers and management.
• Capable of duties across life cycle (infant to geriatric).
• Driver’s license with clean driving record required.
• Bilingual English and Spanish strongly preferred.
• HIPAA/privacy/security knowledge.
• Computer proficiency.

Responsibilities

• Coordinate medical and specialty care for Family Healthcare patients.
• Facilitate communication among medical, nursing, behavioral health, and social services.
• Promote patient-centered, holistic care and treatment.
• Identify patients with chronic disease risk factors and support proactive care planning.
• Monitor progress toward disease-specific self-management goals.
• Conduct home visits and specialty medical visits as appropriate.
• Maintain patient confidentiality and uphold professional standards.
• Support mission, vision, values in interactions.
