Qualifications

• Bachelor’s degree in nursing and active Utah RN license.
• Excellent written and verbal communication skills.
• Ability to supervise and direct a care team and coordinate care.

Responsibilities

• Coordinate and provide comprehensive nursing care to patients in home settings.
• Perform patient assessments and develop care plans in collaboration with the team.
• Coordinate with physicians, therapists, and social services for a coordinated plan of care.
• Ensure compliance with agency standards and regulatory requirements.
• Provide patient and family education to improve quality of life and dignity.
• Supervise RN, LPNs, and home health aides to deliver effective care.
• Maintain timely and accurate clinical documentation and OASIS data where applicable.
• Participate in regular team meetings and on-call coverage.
