Job summary

Qualifications

• Graduate of an accredited School of Registered Professional Nurses; BSN preferred, MSN encouraged.
• Current registration to practice as a Registered Nurse in the applicable state.
• Case Management certification encouraged.

Responsibilities

• Develop, implement, and evaluate case management outcomes for defined patient populations.
• Collaborate with a patient care team to coordinate services and ensure continuity of care.
• Educate, consult, and assist in the management of identified patient populations to achieve quality outcomes.
