Job summary

Corewell Health in Dearborn is seeking a part-time nurse (24 hours/week) to lead discharge planning and care coordination for hospitalized patients. The role integrates utilization management with care transitions, working with physicians, payers, and the care team to optimize outcomes.

The ideal candidate has a nursing degree and 2+ years in acute care, with 3–5 years in care management. Michigan RN license at hire is required; benefits are provided.

Qualifications

• Bachelor's Degree in Nursing required.
• Non-BSN RN considered if pursuing BSN within 2 years of hire.
• 2+ years of relevant experience in acute care.
• 3–5 years in care management, utilization review, home care or discharge planning.
• RN license in Michigan at hire

Responsibilities

• Coordinate admission, stay and discharge planning to ensure medical necessity.
• Review cases for appropriateness and manage a care management caseload.
• Engage with payers, physicians, and care teams to authorize care and transitions.
• Participate in readmission reduction and ensure seamless transitions to lower levels of care.
• Document care plans, discharge plans, and interventions per policy
