Job summary

Corewell Health's Wayne Hospital in Michigan is seeking a part-time nurse for care management, utilization review and discharge planning. Under general direction, you will coordinate admission decisions, promote safe transitions and improve cost effectiveness across the care continuum.

You will manage a caseload, collaborate with physicians, payers and the care team, document reviews and provide patient-centered planning to support high-quality outcomes.

Qualifications

• Bachelor's degree in nursing from an accredited school.
• Three to five years’ experience in care management, utilization review, home care or discharge planning.
• Two to five years’ experience in acute care settings.

Responsibilities

• Identify patients needing care management and discharge planning.
• Manage a caseload for utilization management and care coordination during hospital stay.
• Coordinate care and plan safe transitions to appropriate post-acute settings.
• Collaborate with physicians, payers and staff to obtain authorization and reimbursement.
• Communicate with patients and families to facilitate understanding of coverage and care plans.
