Happy to Help Caregiving - East Idaho is seeking a compassionate RN to provide professional nursing oversight for home-based care in Idaho Falls. You will coordinate care, perform assessments, and guide caregivers to ensure safe, dignified support for veterans and seniors.

Qualifications

• Active RN license in Idaho is required.
• Graduate of an accredited nursing program.
• Valid driver’s license and reliable transportation.
• Must be able to pass a criminal background check.
• Strong assessment, documentation, and care coordination skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently while collaborating with clients, families, and healthcare providers.
• Strong organizational skills and attention to detail.
• Previous home health, hospice, case management, or community-based nursing experience is preferred but not required.

Responsibilities

• Conduct client assessments and reassessments to determine care needs and eligibility for services.
• Develop, review, and update individualized care plans.
• Supervise and support caregivers to ensure quality care and compliance with care plans.
• Provide nursing oversight and clinical guidance to clients, families, and caregiving staff.
• Monitor client health status and identify changes that may require intervention or additional services.
• Coordinate care with physicians, family members, case managers, and other healthcare professionals.
• Educate clients and families on health conditions, safety concerns, and care recommendations.
• Perform nursing visits and maintain accurate documentation in accordance with agency policies.
• Review incident reports, client concerns, and caregiver observations to ensure appropriate follow-up.
• Assist with care transitions and ensuring continuity of care.
• Support client independence, dignity, personal preferences, and overall well-being through compassionate care coordination.
