Job summary

UnityPoint Health's Allen Hospital in Waterloo, IA is seeking an RN Care Coordinator on a part-time, weekend package. This role coordinates patient care across the acute care continuum, conducts admissions and ongoing assessments, and leads discharge planning with the care team.

The ideal candidate will be a licensed RN in Iowa with strong communication and care coordination skills, able to collaborate with physicians, social services, and utilization management to promote safe transitions and

Qualifications

• RN licensure in Iowa.
• Two years RN experience; three years in a clinical setting preferred.
• Must complete in-hospital orientation and competencies.
• Basic computer course completion required.

Responsibilities

• Develops a plan of care including discharge planning with patient/family.
• Facilitates communication with providers and healthcare team on care status and goals.
• Coordinates discharge planning and referrals to social services.
• Educates patients and families and addresses psychosocial barriers.
• Ensures documentation and utilization management alignment.
