Job summary

The University of Iowa Health Care, Care Coordination Division, is seeking a Nurse Clinician/Nurse Navigator to facilitate care progression across the continuum in collaboration with an interdisciplinary team. The role focuses on transitions of care, discharge planning, and optimizing throughput while ensuring compliance and quality outcomes.

Qualified candidates will have a minimum of 3 years of clinical nursing experience, Iowa license, and experience with discharge planning and EPIC.

Qualifications

• Minimum of 3 years of recent clinical nursing experience
• Current license to practice nursing in Iowa, or ability to obtain licensure by date of hire
• Experience working with an interdisciplinary team
• Excellent organizational skills, critical thinking and problem-solving skills
• Excellent written and verbal communication skills
• Proficiency in computer applications including Microsoft Excel, Word, and PowerPoint
• Two years recent medical/surgical inpatient experience with care coordination/case management
• Demonstrated expertise with patient/family discharge planning process
• Professional Master of Nursing and Healthcare Practice (MNHP), MSN/Clinical Nurse Leader or a Master’s Degree in Nursing (MSN, MA)
• Certification in case management
• Previous experience with EPIC

Responsibilities

• Perform transition planning activities at the onset and throughout the patient’s hospitalization, creating and updating personalized transitional care plans.
• Complete initial assessment or ensure completion of all inpatient/observation patients to identify barriers impacting length of stay and discharge planning.
• Collaborate with the patient’s provider and health care team to manage length of stay and determine appropriate level of care for transition planning.
• Assist in discharge planning (referrals) for post-acute facilities, home care, hospice, etc.
• Articulate and communicate plan of care to care team and patient/caregiver to enhance satisfaction.
• Intervene to maintain care progression when deviations occur.
• Participate in multidisciplinary huddles and communicate plans including estimated LOS, barriers, and disposition.
• Provide assessment, counseling, information, referrals, and resource assistance to patients/families as needed.
• Identify high-risk social situations to coordinate resources for follow-up care.
• Arrange community services before discharge to meet post-discharge needs and document patient choices.
• Participate in Care Coordination initiatives or other departmental/organizational projects.
• Perform other duties as assigned to fulfill departmental objectives.
