Job summary

Wesley Enhanced Living in Philadelphia, PA, is seeking a part-time Registered Nurse Assessment Coordinator (RNAC) to perform initial assessments and periodic re-evaluations of residents in our Skilled Nursing facility.

The RNAC will lead interdisciplinary care conferences, update care plans, coordinate case management, and ensure accurate MDS coding and clinical documentation. Requires PA RN license, OBRA knowledge, 2+ years' experience; AANAC preferred; CPR current.

Qualifications

• Minimum 2 years related nursing experience.
• Knowledge of OBRA regulations and PA state requirements.
• Current PA RN licensure and CPR certification.
• AANAC certification preferred within 12 months.

Responsibilities

• Ensure accuracy of MDS coding to maximize reimbursement and patient care.
• Maintain accurate clinical paperwork and documentation.
• Serve as Nursing Educator to train staff on documentation.
• Lead nursing leadership standups and interdisciplinary collaboration.
• Manage case management and coordinate with payers and social workers.
• Oversee records distribution to payers and conduct reviews.
• Jointly own care conferences with social workers.
• Perform additional duties as required.
