Job summary

Great Lakes Management is seeking a Part-time Resident Assistant in Plymouth, MN (7:00am-3:00pm or 3:00pm-11:00pm). The Resident Assistant provides hands-on care, communicates with residents and families, and supports dining and activities per each resident’s service agreement.

Responsibilities include assisting with ADLs, promoting independence, offering physical and emotional support, and coordinating with the RN/case manager while maintaining accurate documentation.

Qualifications

• Six months of experience in long term care or assisted living residence.
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (Preferred).
• Ability to make responsible choices in residents' best interests.

Responsibilities

• Assist residents with ADLs as indicated in the service plan.
• Help residents reach and maintain highest possible level of independence.
• Provide physical and emotional support to residents.
• Assist with treatments as delegated by the RN per the service plan.
• Observe residents and report changes to the case manager.
• Interact with residents to provide reassurance and engagement.
• Complete all required daily documentation.
