Job summary

Kern River Transitional Care Center in Bakersfield, CA is seeking a friendly, organized Part-Time Admissions Coordinator to join our team. The role supports the admissions process and serves as a primary contact for residents, families, referral sources, and staff.

The position involves coordinating admissions, gathering documents, communicating with hospitals and discharge planners, and maintaining accurate records.

Qualifications

• High school diploma or equivalent is required or preferred.
• Experience in healthcare Admissions, administrative work, or customer service is preferred.
• Experience in skilled nursing or long-term care is a plus.
• Basic computer and office skills are required.

Responsibilities

• Assist with coordinating resident admissions, transfers, and inquiries.
• Communicate with residents, families, hospitals, discharge planners, case managers, and other referral sources.
• Gather and organize required admission documents and resident information.
• Prepare admission paperwork and ensure forms are completed accurately.
• Coordinate resident arrival information with nursing and other departments.
• Welcome new residents and families to the facility and assist with the admission process.
• Provide information regarding facility services, policies, and procedures.
• Maintain accurate admission records, logs, and documentation.
• Enter and update resident information in facility systems as required.
• Answer admission-related calls and respond to inquiries professionally.
• Maintain communication with referral sources regarding prospective admissions.
• Assist with verifying information prior to admission.
• Maintain confidentiality of resident information and PHI.
• Develop positive relationships with residents, families, staff, and referral partners.
• Provide excellent customer service and represent the facility professionally.
• Assist with administrative and clerical duties as assigned.
