Qualifications

Responsibilities

  • Assist with coordinating resident admissions, transfers, and inquiries.
  • Communicate with residents, families, hospitals, discharge planners, case managers, and other referral sources.
  • Gather and organize required admission documents and resident information.
  • Prepare admission paperwork and ensure required forms are completed accurately.
  • Coordinate resident arrival information with nursing and other appropriate departments.
  • Welcome new residents and families to the facility and assist with the admission process.
  • Provide information regarding facility services, policies, and procedures.
  • Maintain accurate admission records, logs, and documentation.
  • Enter and update resident information in facility systems as required.
  • Answer admission-related telephone calls and respond to inquiries in a professional and timely manner.
  • Maintain communication with referral sources regarding prospective and pending admissions.
  • Assist with verifying that necessary information is available prior to admission.
  • Maintain confidentiality of all resident information and protected health information.
  • Develop positive working relationships with residents, families, staff, and community referral partners.
  • Provide excellent customer service and represent the facility in a professional manner.
  • Assist with administrative and clerical duties as assigned.

Admissions Coordinator – Part-TimeKern River Transitional Care Center is seeking a friendly, organized, and dependable Part-Time Admissions Coordinator to join our team. The Admissions Coordinator helps facilitate the admission process and serves as an important point of contact for residents, families, referral sources, and facility staff.Pay Range: $17.58–$17.60 per hourGeneral PurposeThe Admissions Coordinator assists with the day-to-day admissions process to ensure new residents and their families receive a welcoming, organized, and positive experience.
