Job summary

Bluebonnet Trails Community Services in Texas is seeking a part-time Transportation Coordinator to support grant-funded transportation initiatives, coordinate between providers and the agency, and report program data. Responsibilities include daily support for ride referrals, monitoring eligibility, tracking utilization, completing monthly service reports, and maintaining data in Excel and the EHR.

Qualifications

• Must have a high school diploma or equivalent.
• Must have intermediate experience with Excel spreadsheets and reports and be comfortable managing data.
• Applicant must have knowledge of the Bluebonnet Trails geographical service area, including all available transportation resources.
• Must have Texas Driver’s License and maintain approved driving status throughout employment.

Responsibilities

• Daily support for transportation services and coordination.
• Monitor all ride request referrals for appropriateness and eligibility.
• Track service utilization and service data and report back to funding entities.
• Participate in quarterly transportation service meetings with internal and external partners.
• Complete at least monthly service reports and track data for review.
• Create basic contact notes in the EHR related to transportation coordination.
• Handle program billing and service reports in Excel.
• Answer the phone in a courteous and helpful manner.
• Work with multiple agencies and individuals; coordinate with staff.
• Operate general office equipment and perform clerical data-entry tasks in multiple agency software (e.g., SmartCare; Teams; Excel).
• This is a part-time position, 20 hours per week, work primarily from home.
