Job summary

Choice Health at Home is seeking a part-time Physical Therapist Assistant (PTA) in Westminster, CO to support licensed therapists in delivering in-home rehabilitation. The PTA will implement treatment plans, monitor progress, and adapt activities to improve mobility and independence for patients in their homes.

The role requires successful completion of an accredited PTA program, state licensure, CPR certification, and a valid driver’s license.

Qualifications

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid PTA license
• 1 year of home health experience preferred.
• AHA CPR certification
• Valid Driver's License
• Valid Auto Insurance
• HomeCare HomeBase experience preferred.

Responsibilities

• Assist physical therapists in executing individualized treatment plans for patients with musculoskeletal and neurological conditions.
• Conduct therapeutic exercises, manual therapy techniques, and modalities under supervision.
• Monitor and document patient responses to treatment, reporting progress and concerns to the supervising therapist.
• Educate patients and families on exercises and use of assistive devices to prevent injury.
• Maintain a safe and clean treatment environment and sanitize equipment.
