CenterWell Home Health in Denver is seeking a Licensed Physical Therapy Assistant to implement the physical therapy plan in patients' homes under the supervision of a Physical Therapist. Requirements include a valid state PTA license, CPR, a driver’s license with reliable transportation, and experience in geriatric care.

Qualifikationen

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric changes.
• Read, write, and speak fluent English.

Aufgaben

• Administer the physical therapy treatment plan to patients in their homes.
• Administer non-complex manual therapy exercises and modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in patient care.
