Concept Rehab & Engage Consulting in Delaware, Ohio is seeking a Part-Time Physical Therapist to join a Skilled Nursing Facility team. You will evaluate and treat adults and seniors recovering from illness, injury, or surgery, developing goal-based care plans within an interdisciplinary setting.

This role emphasizes evidence-based treatment, patient safety, and collaborative care with physicians, nurses, and therapists. A state license and commitment to quality geriatric rehab are essential.

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Physical Therapy (BS, MS, MPT, or DPT).
• Current Ohio Physical Therapist license in good standing.
• Strong clinical reasoning and patient care skills.
• Excellent communication and documentation abilities.
• Ability to work collaboratively within an interdisciplinary rehabilitation team.

Responsibilities

• Perform physical therapy evaluations within established timelines following physician referral.
• Develop individualized plans of care based on patient needs and goals.
• Provide evidence-based physical therapy treatment interventions.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete discharge planning and recommendations.
• Communicate effectively with physicians, nurses, rehabilitation staff, patients, and family members.
• Educate patients and caregivers regarding treatment goals, safety, and home or maintenance programs.
• Supervise PTAs as required by state regulations.
