Job summary

All Care Health Solutions seeks a Psychiatric Registered Nurse to join our home health team in Eagle, Idaho, on a part-time basis. The RN will assess and monitor geriatric patients with behavioral health needs and develop individualized care plans.

Responsibilities include medication management, education for patients and families, crisis prevention, and coordination with physicians and the interdisciplinary team to ensure safe, coordinated care.

Qualifications

• Active Idaho RN license in good standing.
• Master's degree with a specialty in mental health nursing or equivalent.
• Bachelor's degree in nursing with 1 year of recent psychiatric hospital, psychiatric home care, or other outpatient psychiatric experience OR Associate's degree in nursing with 2 years of related experience.

Responsibilities

• Perform comprehensive psychiatric and nursing assessments to identify mental health, behavioral, cognitive, and psychosocial needs.
• Monitor and manage patients with depression, anxiety, bipolar disorder, schizophrenia, dementia-related behaviors, and other psychiatric conditions.
• Provide medication management including assessment of effectiveness, side effects, adherence, and coordination with prescribers.
• Evaluate coping skills and emotional functioning to inform care planning.
• Develop individualized care plans addressing psychiatric and medical needs.
• Educate patients, families, and staff on mental health diagnoses, medications, symptoms, and interventions.
• Identify changes in mental status or safety concerns and coordinate timely interventions.
