You will perform routine rounds, assess and manage mental health conditions, and collaborate with interdisciplinary teams to support residents’ emotional well‑being. The role emphasizes proactive, value‑based care and strong partnerships with facilities.

Qualifications

• Current NP or PA licensure in the state of practice.
• Experience in geriatrics, family medicine, or psychiatry preferred.
• Exceptional patient-centered service skills and ability to build trust with residents and staff.

Responsibilities

• Provide behavioral health and psychiatric medication management within skilled nursing and senior living communities.
• Perform routine patient rounding and ongoing mental health management.
