Job summary

PMCH is seeking a Part-Time Program Support Specialist for Family Connects New Jersey (FCNJ) in Essex County. The role provides administrative, logistical, and operational support to ensure successful program delivery, coordinating activities and maintaining records.

Responsibilities include outreach, hospital bedside recruitment, mentoring new staff, data entry in Salesforce, and participation in weekly nursing staff meetings.

Qualifications

• Bachelor's Degree in Public Health or related field preferred.
• Minimum 3 years of experience in community outreach, public health, or healthcare.
• Experience working with diverse populations and organizations.
• Proficient with Microsoft Office Suite.

Responsibilities

• Conduct outreach and scheduling of appointments with families through collaboration with community-based programs.
• Participate in bedside recruitment at hospitals, clinics, and birthing facilities.
• Assist with mentoring, precepting, and shadow visits for new staff.
• Complete appointments and patient follow-up data using MS Office Suite and Salesforce.
• Document outreach attempts with families using web-based data systems.
• Monitor FCNJ email and phone accounts to respond to inquiries.
• Conduct post-visit calls with clients to assess services and referrals.
• Participate in weekly team meetings with nursing staff.
• Develop and maintain community/provider referral sources with nursing staff.
