Job summary

Evolve Therapy Services in Madisonville, TN invites a licensed Physical Therapist to join our in-house Skilled Nursing Facility team on a part-time or PRN basis. You will provide high-quality physical therapy services to residents in accordance with current standards, procedures, and CMS expectations.

The role emphasizes evaluation, treatment planning, documentation accuracy, and collaboration with families and the interdisciplinary team.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound clinical judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Conduct screening of residents to determine intervention needs and treatment.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and recommendations to residents and the interdisciplinary team.
