Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist to work in-house at a Skilled Nursing Facility in Madisonville, TN. Part-time or PRN hours allow you to choose your schedule while delivering high-quality resident care.

You will evaluate residents, develop treatment plans, and collaborate with an interdisciplinary team. Proficiency in NetHealth EMR and commitment to ongoing licensure are essential.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Ability to develop and implement treatment plans and communicate findings.

Responsibilities

• Provide physical therapy services to residents per standards, procedures and productivity goals.
• Document and bill therapy services per procedures.
• Evaluate residents to inform treatment planning and progress.
• Develop short and long-term treatment goals and methods to achieve them.
• Communicate evaluation findings, plans, and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
