Job summary

CareMed Health Solutions seeks a Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities in Fort Collins, CO. You will manage acute changes, plan advanced care, and prescribe medications per Colorado law.

Responsibilities include coordinating tests, communicating with nursing staff, and collaborating with PCPs within an interdisciplinary team. Colorado NP license, Board Certification, and DEA licensure are required.

Qualifications

• Current, unrestricted NP license in the state of Colorado.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
