Job summary

CareMed is seeking a capable Nurse Practitioner to deliver primary and preventive care to residents in skilled nursing facilities. You will manage acute changes, participate in advanced care planning, and coordinate chronic care management with an interdisciplinary team.

You will preside over medications within KY law and collaborate closely with primary care physicians to ensure seamless patient care. The role emphasizes long-term care excellence, EMR proficiency, and the ability to support

Qualifications

Active NP license in Kentucky and board certification.
DEA licensure is required.
Experience in long-term care settings.
Ability to coordinate with an interdisciplinary care team.
Experience with EMR systems.
Clinical background in adult, family or geriatric care.
Ability to manage patients with advanced illness.
Knowledge of prescribing within state law.

NP ROLE AND RESPONSIBILITIES:

Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
Manage Residents with acute change in condition.
Advanced care planning and chronic care management.
Prescribe appropriate medications, including controlled substances in accordance with KY Law.
Order, review and/or follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
and/or the care plan.
Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

Current, unrestricted NP license in the state of Kentucky.
Board Certification.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience working in a long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team to coordinate patient care.
Psych experience a plus
