CareMed is seeking a Nurse Practitioner to provide primary and preventive care for residents in skilled nursing facilities. You will manage acute changes, coordinate care plans, and collaborate with primary care physicians.

This role requires current NP license, board certification, DEA licensure, and experience in geriatric or adult care in long-term care settings. EMR proficiency and strong teamwork are essential; a competitive benefits package is offered.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.

• Board Certification.

• Ability to work with an EMR.

• Ability to manage patients with advanced illness and multiple chronic conditions.

• Ability to work within an interdisciplinary care team to coordinate patient care.

• Psych experience a plus
