Job summary

Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care to residents in long-term care settings in Chisholm, MN. This part-time role emphasizes meaningful patient relationships and collaborative care.

You will perform exams, manage chronic and acute conditions, order tests, and educate residents and families about treatment options. Minnesota licensure and LTC experience are required.

Qualifications

• Active and unrestricted NP/PA license.
• State license in good standing in Minnesota.
• Minimum 6 months of long-term care or skilled nursing facility experience.
• Strong communication, clinical, and critical thinking abilities.
• Passion for elder care and patient-centered healthcare.

Responsibilities

• Provide primary care services in skilled nursing and long-term care facilities.
• Perform physical exams and review medical history.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
