Job summary

Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner/Physician’s Assistant to deliver on-site care in long-term care facilities in Northfield, MN. This part-time role (2 days/week) emphasizes meaningful patient relationships and collaborative care.

Requirements include an active Minnesota NP/PA or physician license, 6 months in long-term care, and strong communication and critical-thinking skills. A passion for elder care is essential.

Qualifications

• Active and unrestricted NP/PA or physician license.
• State-specific license in Minnesota in good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication and critical-thinking skills.
• Interest and passion for elder care and person-centered healthcare.

Responsibilities

• Provide primary care services to residents in long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and labs.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Maintain a safe, respectful, and supportive environment for residents.
