Job summary

Infinite Medical P.C. seeks a Primary Care NP/PA to provide on-site care for residents in long-term care settings in Littlefork, MN. This part-time role focuses on meaningful patient relationships and collaborative care within a facility team.

The provider will perform exams, manage chronic and acute conditions, order tests, and develop personalized care plans, with a supportive, patient-centered environment and opportunities for professional growth.

Qualifications

• Active and unrestricted NP, PA, or physician license.
• State license in good standing in Minnesota.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical, and critical-thinking skills.
• Passion for elder care and person-centered healthcare.
• Ability to stand/walk for extended periods and lift up to 20 lbs; computer use.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for residents
