Job summary

Northwestern Medicine is seeking a part-time Physical Therapist for 20 hours per week at Lake Forest Hospital. The role requires weekend and holiday rotation and includes direct patient care and teaching responsibilities under NM guidelines.

The incumbent will maintain patient plans of care, collaborate with PT/PTA team, participate in committees, and contribute to department processes. Adherence to all ethical and compliance standards is expected.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Licensed Physical Therapist in State of Illinois required.
• BLS from American Heart Association upon hire.
• Masters or Doctoral Degree in Physical Therapy preferred.

Responsibilities

• Demonstrates NM values of teamwork and excellence in daily work.
• Provides direct patient care and participates in teaching and committees.
• Maintains patient plans of care with treatment collaboration and documentation.
• Completes daily EMR notes and discharge planning as required.
• Participates in continuous improvement and cost-reduction efforts.

Bachelor’s Degree in Physical Therapy
