Responsibilities include evaluating and re-evaluating patients, documentation in EMR, and contributing to departmental process improvement while upholding NM values.

Qualifications

• Bachelor’s Degree in Physical Therapy is required.
• Licensed Physical Therapist in State of Illinois.
• BLS certification required.
• Masters or Doctoral Degree in Physical Therapy preferred.

Responsibilities

• Demonstrates NM values in daily work and interactions.
• Provides direct patient care and contributes to treatment planning.
• Educates patients and families as part of care.
• Collaborates with PT/PTA team to maintain plans of care.
• Documents progress and maintains EMR with daily notes.
• Participates in department processes and quality improvement.
• Supports cross-department teamwork and continuity of care.
• Assists in student clinical education and mentorship.
