Job summary

Tampico Healthcare Center, a 128-bed skilled nursing rehabilitation center in Walnut Creek, CA, seeks a Part-Time Physical Therapist to join our in-house rehab team. You will assess residents, plan and provide PT interventions, and collaborate with an interdisciplinary team to ensure high-quality care.

Responsibilities include supervising assistants, coordinating care, and contributing to discharge planning while adhering to licensure requirements.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current, active Physical Therapy License.
• Experience in post-acute or long-term care preferred.
• Knowledge of PT standards and guidelines.

Responsibilities

• Perform PT assessments and develop individualized treatment plans.
• Supervise PT Assistants, provisional licensees, students, and support staff.
• Identify referrals to other disciplines to address comprehensive care needs.
• Demonstrate competent technical PT skills per current standards of practice.
• Communicate changes in residents' conditions and participate in discharge planning.
• Prescribe necessary durable medical equipment to facilitate independence.
• Maintain accurate billing records and clinical documentation.
• Safeguard patient confidentiality.
