Job summary

Kern River Transitional Care in Bakersfield, CA is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team on a part-time basis. You will evaluate residents, design personalized plans, and provide skilled PT services to improve mobility and independence.

The role emphasizes collaboration with an interdisciplinary team, documentation accuracy, and adherence to safety standards. Current CA PT license and CPR/BLS are required; new grads encouraged to apply.

Qualifications

• Graduate of an accredited physical therapy program.
• Current and unrestricted California Physical Therapist license required.
• CPR/BLS certification required.
• New graduates are encouraged to apply.
• Strong clinical assessment, communication, documentation, and time-management skills.
• Ability to work independently and collaboratively as part of an interdisciplinary team.
• Knowledge of PointClickCare or another EMR system preferred.
• Compassionate and resident-centered approach to care.

Responsibilities

• Evaluate residents' mobility, strength, balance, coordination, ROM and functional abilities.
• Develop individualized treatment plans based on residents' needs and goals.
• Provide skilled PT services to improve mobility, independence, safety and function.
• Help residents regain walking, transfers, balance and daily activities.
• Establish measurable goals and adjust plans based on progress.
• Provide therapeutic exercise, gait training, balance, transfer training and interventions.
• Train residents in safe use of walkers, wheelchairs, canes and adaptive equipment.
• Educate residents, families, staff on treatment plans and mobility techniques.
• Complete evaluations, progress notes, discharge summaries and required documentation.
• Collaborate with physicians, nurses, occupational therapists, SLPs and the team.
• Participate in care conferences, discharge planning and quality-improvement.
• Supervise PTAs and rehab staff as permitted by law and policy.
• Maintain privacy and confidentiality of health information.
• Follow professional standards, policies and regulatory requirements.
• Perform additional duties as assigned.
