Job summary

Theracare in Portage, IN is seeking a motivated Physical Therapist to deliver exemplary care to our adult population in skilled nursing and home health settings. This Part-time/PRN role offers flexible scheduling and the opportunity to work closely with a dedicated therapy team.

You will evaluate and treat patients, adhere to physician orders, and contribute to quality outcomes, with PTO available for part-time staff and access to regional management support.

Qualifications

• Evaluate and treat patients per physician orders using varied therapy procedures.
• Collaborate with therapy team to coordinate care and patient progress.
• Willingness to provide therapy services in multiple settings (SNF, outpatient, LTC, home health).

Responsibilities

• Deliver physical therapy services in Skilled Nursing Facilities, Outpatient, Long Term Care or Home Health Care.
• Collaborate with physicians, therapists and nursing staff to optimize patient outcomes.
• Maintain documentation and ensure compliance with accreditation standards.
