Job summary

Enhabit Home Health & Hospice is seeking a part-time Physical Therapist to provide skilled therapy to patients at home. The role focuses on improving mobility, functional independence, and self-care through therapeutic interventions and education.

The position offers a 24-hour work week over four days, with eligibility for their 60% paid days off program and a supportive, collaborative team environment.

Qualifications

• Must be currently licensed as a Physical Therapist in the state of employment.
• Must possess valid state driver’s license.

Responsibilities

• Examine and treat patients to improve mobility and function.
• Administer skilled physical therapy services per physician orders and plan of care.
• Collaborate with care team and educate family and caregivers.
• Support specialty programs such as Balance and Fall Prevention, Spine Safety, and joint replacement programs.
