Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled care to patients in their homes. The role focuses on improving mobility and functional independence through physical modalities and customized plans of care.

Responsibilities include patient assessment, delivering therapy under physician orders, and educating families and care teams. The position operates under supervision of the Branch Director in a supportive, growth-oriented environment.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Assist patients to improve mobility and daily living activities.
• Administer skilled care under physician orders and plan of care.
• Educate the patient and family, and coordinate with the health care team.
• Work under the direction and supervision of the Branch Director.
