Job summary

Qualifications

• Graduate of an accredited Physical Therapy program per APTA CAPTE.
• Current, active Physical Therapy license required.
• Experience in post-acute or long-term care preferred.

Responsibilities

• Perform comprehensive PT assessments and create individualized treatment plans.
• Supervise Physical Therapy Assistants and students in accordance with licensure requirements.
• Refer to other disciplines when integrated care is needed.
• Provide documentation and billing records for services rendered.
• Collaborate with the rehab team to support discharge planning and mobility goals.
