Job summary

River Terrace Retirement Community in Bluffton, IN, and Kendallville Manor in Kendallville, IN, seek a dedicated Part-Time Physical Therapist to provide rehabilitation services across both facilities.

This role offers approximately 20–25 hours per week split between the two locations, focusing on residents' mobility, independence, and quality of life. Ideal candidate is passionate about patient-centered care for older adults and willing to travel between sites.

Qualifications

• Licensed Indiana Physical Therapist in good standing.
• Graduate of an accredited Physical Therapy program.
• Experience in skilled nursing, long-term care, rehabilitation, or geriatric care preferred.

Responsibilities

• Evaluate residents' PT needs and create individualized treatment plans.
• Provide PT interventions to improve strength, balance, mobility, and ROM.
• Assist residents with transfers, ambulation, and activities of daily living.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with nursing, OT, SLP, and other team members to coordinate care.
• Educate residents, families, and caregivers on exercises, mobility techniques, and safety.
