Job summary

River Terrace Retirement Community in Bluffton, IN, and Kendallville Manor in Kendallville, IN, are seeking a dedicated Part-Time Physical Therapist (PT) to provide rehabilitation services across both facilities. Qualifications

• Current Indiana Physical Therapist (PT) license in good standing.
• Graduate of an accredited Physical Therapy program.
• Previous experience in skilled nursing, long-term care, rehabilitation, or geriatric care preferred.
• Strong clinical assessment, communication, and interpersonal skills.
• Ability to travel between the Bluffton and Kendallville facilities.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to compassionate, patient-centered care.

Responsibilities

• Evaluate residents' physical therapy needs and develop individualized treatment plans based on assessments and functional goals.
• Provide PT interventions to improve strength, balance, mobility, coordination, and range of motion.
• Assist residents in improving functional independence with transfers, ambulation, and activities of daily living.
• Monitor resident progress and modify treatment plans as needed to support optimal outcomes.
• Collaborate with nursing staff, Occupational Therapists, Speech Therapists, and other interdisciplinary team members.
• Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, and safety precautions.
• Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and in a timely manner.
• Ensure all therapy services comply with applicable state and federal regulations and professional practice standards.
• Promote resident safety and maintain a supportive, encouraging rehabilitation environment.
