Job summary

River Terrace Retirement Community in Bluffton, IN, and Kendallville Manor in Kendallville, IN, seek a dedicated Part-Time Physical Therapist to provide rehabilitation across both facilities. Approximately 20–25 hours per week split between the two locations, with scheduling coordinated between facilities.

The ideal candidate will be passionate about patient-centered care for older adults, skilled in evaluating PT needs, and collaborating with an interdisciplinary team to support residents’

Qualifications

• Indiana PT license required and in good standing.
• Graduate of an accredited Physical Therapy program.
• Experience in skilled nursing/geriatric care preferred.
• Strong clinical assessment and interpersonal skills.
• Ability to travel between Bluffton and Kendallville facilities.

Responsibilities

• Evaluate residents' PT needs and develop treatment plans.
• Provide PT interventions to improve strength, balance, mobility, ROM.
• Assist with transfers, ambulation, and ADLs.
• Monitor progress and modify plans as needed.
• Collaborate with nursing and other therapists to coordinate care.
• Educate residents and families on exercises and safety.
• Document evaluations and progress notes.
• Ensure compliance with regulations and professional standards.
• Promote resident safety and a supportive rehab environment.
