The role emphasizes collaboration with physicians and nurses, adherence to CMS/DEA/FDA standards, and ongoing training with advancement opportunities in a growing company.

Qualifications

• Bachelor’s Degree in Pharmacy required or Pharm D preferred.
• Licensed pharmacist in the state of practice (OK).
• Experience in long-term care is desirable.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff on therapy changes.
• Maintain patient/resident profiles with medical history, labs, and current medications.
• Ensure monitoring tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses and other professionals for clinical support.
• Document all communications and interventions in medical records.
• Enforce policies for safe handling, dispensing and oversight of medications.
• Ensure compliance with state and federal regulatory requirements.
• Report medication-related errors to Pharmacy Manager.
• Educate clinical team on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
