Job summary

Jobtailor is seeking a qualified pediatric registered nurse to provide comprehensive home hospice care and support to children and families in Falls Church, VA.

Qualifications

  • Current RN license in the state where the employee will be working.
  • Minimum of two years of pediatric nursing experience.
  • Hospice or hospital experience preferred.
  • Experience with EMR/EHR systems.
  • Valid driver's license and reliable transportation.

Responsibilities

  • Coordinate Plans of Care with the Interdisciplinary Team (IDT).
  • Provide direct clinical, palliative, supportive, psychosocial, spiritual and cultural care to pediatric patients and families.
  • Document patient/family contacts and care in the EMR.
  • Visit patients at home and coordinate with hospitals, physicians, and families.
  • Communicate clinical information using SBAR and coordinate after-hours support.

Job description

  • Assess and identify patient/family needs using the nursing process
  • Coordinate individualized Plans of Care with the Interdisciplinary Team (IDT)
  • Provide direct clinical, palliative, supportive, psychosocial, spiritual, cultural, physical and biological care to pediatric patients and families
  • Provide reassurance and support to patients and families by phone and coordinate in-person visits when needed
  • Document patient/family contacts and care in the EMR
  • Complete initial and semi-annual assessments for Company services
  • Explain services, disease processes, decline, prevention, palliative interventions, caregiving, dying process, concurrent care, age-appropriate care and safety practices
  • Provide information to physicians and IDT members and initiate Plans of Care
  • Initiate skilled nursing interventions to prevent complications, alleviate symptoms and maximize comfort
  • Obtain physician orders and complete documentation according to Company policy
  • Facilitate referrals to service lines and coordinate with hospitals, physicians, facilities, patients, families and the public
  • Communicate clinical information using SBAR and discuss potential needs with after-hours staff
  • Develop relationships with case managers, physicians and other providers
  • Manage direct patient and family care as part of the IDT, including follow-up
  • Conduct home visits to assess home safety, medication and nutritional compliance, DME compliance and ability to live safely in the community
  • Report changes in patient condition to IDT members and other health professionals
  • Participate in evaluation of hospice referrals/admissions for level-of-care appropriateness
  • Attend daily IDT collaboration meetings and present oral and written reports
  • Supervise care provided by Community Health Workers and Home Health Aides as requested
  • Report to a company-designated location during emergencies and potentially stay overnight
  • Perform other duties as assigned
Requirements
  • Current RN license in the state where the employee will be working
  • Minimum of two years of pediatric nursing experience
  • Hospice or hospital experience preferred
  • Previous experience with an EMR/EHR system
  • Valid driver's license and automobile insurance per Company policy
  • Reliable transportation to meet the visit schedule
  • Ability to use equipment with visual and auditory mechanisms
  • Ability to communicate effectively in English, verbally and in writing
  • Ability to visit participants in their homes for assessments
  • Ability to perform essential job functions and physical requirements, including lifting patients/equipment, bending, pushing/pulling and kneeling, with or without reasonable accommodation
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross
  • Satisfactory completion of competency requirements
  • Consent to drug and/or alcohol testing after a conditional offer and ongoing compliance with the Drug-Free Workplace Policy
